The investigation began on January 14, 2015, when the Rainy River District Ontario Provincial Police (OPP) received information from La Verendrye Hospital in Fort Frances regarding a suspicious death along with a theft.
As a result of a lengthy investigation, members of the Rainy River District OPP Crime Unit, under the direction of Detective Staff Sergeant Kevin Veillieux of the Criminal Investigation Branch, have arrested and charged Lindsey COYLE, 34-years-old, of Fort Frances with the following Criminal Code offences:
- Second Degree Murder,
- Criminal Negligence Causing Death,
- Breach of Trust,
- Utter Forged Documents.
The investigation stems from her employment as a Registered Practical Nurse (RPN) at the La Verendrye Hospital in Fort Frances along with a subsequent review of the death of Hermina Fletcher by the Office of the Chief Coroner of Ontario (OCCO).
